A complete guide to Viagra Connect for UK patients

DoktorABC editorial team
Accurate, up-to-date health information written by our editorial team and reviewed by UK-registered prescribers.

Viagra Connect is the only erectile dysfunction medicine available from UK pharmacies without a prescription. For men who find discussing ED with their GP difficult, or who simply want a more direct route to treatment, its pharmacy availability has made a practical difference since it went on sale in March 2018. This guide covers what it is, how it works, the two formulations now available, who it is suitable for, and what to expect from treatment.

What is Viagra Connect?

Viagra Connect is a pharmacy medicine (P medicine) containing 50 mg of sildenafil citrate, a PDE5 inhibitor licensed for the treatment of erectile dysfunction in adult men. It is manufactured by Viatris Products Limited and is available in two formulations: a film-coated tablet and an orodispersible film (Viagra Connect Melts).

The MHRA reclassified Viagra Connect from a prescription-only medicine to a pharmacy medicine on 28 November 2017, following assessment by the Commission on Human Medicines and a public consultation. It has been available for purchase from UK pharmacies following a pharmacist consultation since March 2018.

Viagra Connect is licensed for adult men with erectile dysfunction, defined as the inability to achieve or maintain a penile erection sufficient for satisfactory sexual performance. It is not intended for men without erectile dysfunction and is not for use by women or anyone under 18 years of age.

How does Viagra Connect work?

An erection involves a chain of events starting with sexual stimulation. Stimulation triggers the release of nitric oxide in the erectile tissue of the penis (the corpus cavernosum). Nitric oxide activates an enzyme that produces a chemical called cGMP, which causes the smooth muscle in the penile blood vessels to relax and allows blood to flow in, producing an erection.

An enzyme called PDE5 breaks down cGMP, which causes the erection to subside. Sildenafil, the active ingredient in Viagra Connect, is a selective inhibitor of PDE5. By blocking PDE5, sildenafil allows cGMP to accumulate and the erection to be maintained during sexual stimulation.

The critical point, which the SmPC emphasises, is that sildenafil only works when the nitric oxide pathway has been activated by sexual stimulation. It does not cause an erection independent of arousal. Without sexual stimulation, taking Viagra Connect will not produce an erection.

Viagra Connect versus Viagra: what is the difference?

Is Viagra Connect the same as Viagra?

Both contain sildenafil as the active ingredient, and at the 50 mg dose, the clinical effect is the same. They vary in their legal status, available doses, how you access them, and cost.

Feature Viagra Connect Prescription Viagra Generic sildenafil (prescription)
Active ingredient Sildenafil Sildenafil Sildenafil
Available dose 50 mg only 25 mg, 50 mg, 100 mg 25 mg, 50 mg, 100 mg
Legal status Pharmacy medicine (P) Prescription only (POM) Prescription only (POM)
Access route Pharmacist consultation GP or prescriber GP or prescriber
Branded only Yes (Viatris) Yes (Viatris) No, multiple manufacturers
Cost per tablet Higher Higher Considerably lower

The most clinically significant difference is dose flexibility. Viagra Connect is available only at 50 mg. If a different dose is clinically appropriate, for example 25 mg for men on certain other medicines, or 100 mg where 50 mg has proved insufficient, a prescription from a GP or registered prescriber is required.

How long does Viagra Connect take to work?

The SmPC recommends taking Viagra Connect approximately one hour before sexual activity.

In clinical studies, most men taking Viagra Connect on an empty stomach noticed an effect within 25 to 30 minutes, though for some it took closer to the full hour. Taking it approximately one hour before sexual activity gives the medicine enough time to work for most people.

The effect of food:

Food, particularly a high-fat meal, can significantly delay the onset of effect. When the standard film-coated tablet is taken with food, the time to maximum effect is delayed by approximately 60 minutes and peak concentration is reduced by 29%.

For Viagra Connect Melts, the SmPC notes that a significant delay in absorption is expected when taken with a high-fat meal compared to on an empty stomach.

Practically, this means that if you are planning to take Viagra Connect after a large meal, particularly one high in fat, you may need to allow considerably more time than the standard one-hour recommendation, or take the tablet before eating.

How long does the effect last?

Sildenafil is processed and cleared by the body over three to five hours. Clinical studies showed that sildenafil was still able to produce an erection in response to sexual stimulation four to five hours after the dose.

This means the window of opportunity for sexual activity is broadly four to five hours following an effective dose, though individual variation exists. The SmPC is clear that this describes a window of responsiveness to sexual stimulation, not a duration of erection. Erections occur in response to stimulation during this period, not continuously.

Important!

An erection that persists for more than four hours, known as priapism, requires immediate medical attention. Priapism is rare but can cause permanent harm if not treated promptly.

How to take Viagra Connect

Both formulations share the same active ingredient and dose, and the same general guidance applies to both. The following is based on the current SmPCs.

Standard film-coated tablet:

  • Take one tablet with water approximately one hour before sexual activity
  • Maximum one tablet per day
  • Taking with food, particularly a high-fat meal, may delay the effect
  • Do not take more than one tablet in any 24-hour period

Viagra Connect Melts (orodispersible film):

  • Peel open the aluminium pouch carefully (do not cut it)
  • Remove the film with a dry finger and place it on the tongue
  • Allow it to disintegrate; it can be taken with or without water
  • Take it immediately upon removal from the pouch
  • Take on an empty stomach; food, particularly high-fat, will delay absorption
  • Maximum one film per day

Both formulations may need to be tried on several separate occasions before the medicine works as expected for some men. If after several attempts the medicine has not worked, a GP should be consulted.

A note on dose:

Viagra Connect is only available at 50 mg. For some men, such as those taking alpha-blockers or certain other medicines, a lower 25 mg dose may be more clinically appropriate. This is a prescriber decision, not something that can be managed by adjusting Viagra Connect. If you think a different dose may be more suitable for you, speak to a GP.

The two Viagra Connect formulations

Viagra Connect 50 mg film-coated tablet

The original formulation, available since 2018. A blue, rounded diamond-shaped tablet marked with V on one side and 50 on the other. Swallowed whole with water. Available in packs of 2, 4, or 8 tablets.

Viagra Connect Melts 50 mg orodispersible film

A newer formulation, authorised on 5 August 2024. A thin, pale red orodispersible film, approximately 24 mm by 32 mm, individually sealed in aluminium pouches. It is placed on the tongue and dissolves without the need to swallow a tablet, which some men find more convenient or discreet.

Clinical studies confirmed that the orodispersible film is bioequivalent to the film-coated tablet, meaning the two formulations deliver the same amount of sildenafil to the bloodstream. The choice between them is one of personal preference and convenience rather than clinical difference.

Both formulations carry the same contraindications, precautions, and side effect profile.

What to expect from a pharmacy consultation

Because Viagra Connect is a pharmacy medicine rather than a prescription-only medicine, a pharmacist must assess your suitability before supplying it. This is a clinical safeguard, not a formality.

The pharmacist will typically ask about:

  • Your age (must be 18 or over)
  • Your heart health, including whether you have had a recent heart attack or stroke, have unstable angina, or experience chest pain or significant breathlessness with light physical activity
  • The medicines you currently take, particularly nitrates, alpha-blockers, ritonavir, and riociguat
  • Whether you have been previously diagnosed with ED or whether this is a new concern
  • Whether you have any of the conditions that contraindicate use, such as severe liver problems, very low blood pressure, or a history of vision loss in one eye

If the pharmacist identifies a condition that makes Viagra Connect unsuitable, they will advise you to see your GP. The SmPC also recommends that all men with erectile dysfunction consult a doctor within six months for a clinical review of potential underlying conditions, as ED can be associated with cardiovascular disease, diabetes, and other health conditions.

The pharmacy consultation covers the same ground as a clinical assessment conducted by an online prescribing service, which means accessing Viagra Connect through a regulated online provider is a legitimate alternative for men who prefer not to discuss ED with their local pharmacist in person.

Who should not use Viagra Connect?

The following is a summary of the contraindications set out in the SmPC. This is not a complete list, and the pharmacist consultation exists to identify individual circumstances that may affect suitability.

Absolute contraindications:

  • Men taking nitrates in any form, including glyceryl trinitrate (GTN), isosorbide mononitrate, isosorbide dinitrate, nicorandil (which has a nitrate component), or amyl nitrite. The combination can cause a dangerous and potentially life-threatening drop in blood pressure
  • Men taking ritonavir (an HIV medicine)
  • Men taking riociguat (used for pulmonary arterial hypertension)
  • Men with severe cardiovascular disorders, including a heart attack or stroke within the past six months, unstable angina, or severe heart failure
  • Men with severe hepatic impairment
  • Men with low blood pressure (below 90/50 mmHg)
  • Men with known hereditary degenerative retinal disorders such as retinitis pigmentos
  • Men who have previously experienced vision loss in one eye due to non-arteritic anterior ischaemic optic neuropathy (NAION)
  • Men with anatomical deformation of the penis

Men who should consult a GP before use:

  • Men taking alpha-blockers (used for prostate conditions or high blood pressure), as a lower 25 mg prescription dose may be more appropriate
  • Men with severe kidney impairment
  • Men with mild to moderate liver impairment
  • Men taking CYP3A4 inhibitors, such as certain antifungal or antibiotic medicines
  • Men for whom light physical activity, such as walking briskly for 20 minutes or climbing two flights of stairs, causes significant breathlessness or chest pain

Important!

If you experience any sudden loss of vision while taking Viagra Connect, stop using it immediately and contact a doctor. Report any suspected side effects to the MHRA via the Yellow Card scheme.

How to access Viagra Connect through DoktorABC

Viagra Connect is available through DoktorABC following an online consultation. A UK-registered prescriber reviews your information and, if Viagra Connect is clinically appropriate, approves access. Medication is dispensed by a GPhC-registered pharmacy and delivered in discreet packaging.

See our Viagra Connect product page for full details of the consultation process.

Conclusion

Viagra Connect gives men in the UK a direct, accessible route to a clinically well-evidenced treatment for erectile dysfunction. Its reclassification to a pharmacy medicine in 2017 was specifically aimed at reducing the barriers that prevented many men from seeking help, and directing them away from the risks of unregulated online sources.

The medicine works in conjunction with sexual stimulation rather than independently of it, requires some timing consideration around food, and may need to be tried on several occasions before the response is established. For men who need dose flexibility beyond the 50 mg pharmacy option, or for whom the medicine does not work after several attempts, a prescriber consultation is the appropriate next step.

FAQ

Is Viagra Connect the same as Viagra?

Both contain sildenafil as the active ingredient. At the 50 mg dose, the clinical effect is the same. The differences are that Viagra Connect is a pharmacy medicine available without a prescription (following a pharmacist consultation), while prescription Viagra is available at 25 mg, 50 mg, and 100 mg and requires a prescriber.

Can I buy Viagra Connect online?

Yes, from regulated UK online pharmacies that require a clinical assessment before supplying it. Viagra Connect is a pharmacy medicine and cannot be legally supplied without an appropriate pharmacist or prescriber assessment. Do not purchase it from sources that offer it without any suitability check. Always verify that any online pharmacy is registered with the GPhC at gphc.org.uk.

How quickly does Viagra Connect work?

The recommended timing is approximately one hour before sexual activity. In clinical studies in fasted patients, the median onset of a sufficient erection was 25 minutes, with a range of 12 to 37 minutes. A high-fat meal can delay the onset by approximately one hour.

How long does Viagra Connect last?

Sildenafil has a half-life of three to five hours, and clinical studies showed it could still produce erections in response to sexual stimulation four to five hours after dosing. This describes a window of responsiveness to stimulation, not a duration of erection.

What is the difference between Viagra Connect tablets and Viagra Connect Melts?

Both contain 50 mg of sildenafil and are bioequivalent, meaning they deliver the same amount of the active ingredient to the bloodstream. The Melts dissolve on the tongue without needing to swallow a tablet and do not require water. The choice between them is personal preference. Both must be taken on an empty stomach for the best results.

Can I take Viagra Connect if I take blood pressure medication?

It depends on the specific medicine. Men taking nitrates must not use Viagra Connect under any circumstances. Men taking alpha-blockers should consult a GP before use, as a lower prescription dose may be more suitable. Most other blood pressure medicines are not contraindicated, but the pharmacist consultation will assess your individual medicines before supplying.

What should I do if Viagra Connect does not work?

The SmPC advises that some men need to try it several times before finding it effective. If after several attempts it has still not worked, or if ED worsens, a GP should be consulted. Persistent non-response may reflect an underlying health issue or suggest a different dose or treatment approach.

Can Viagra Connect cause side effects?

Yes. The most commonly reported in clinical trials were headache, flushing, dyspepsia, nasal congestion, dizziness, nausea, hot flush, visual disturbance, and blurred vision. Most are mild and short-lived. If you experience sudden vision loss, stop taking Viagra Connect and seek immediate medical attention. Report suspected side effects to the MHRA Yellow Card scheme at yellowcard.mhra.gov.uk.
