How can I get treated for erectile dysfunction?

Erectile dysfunction (ED) is defined as the inability to achieve an erection or to maintain it for long enough to have sexual intercourse. There are many different causes of ED, but ultimately, erectile dysfunction occurs when the flow of blood to the penis is insufficient or the blood leaves the penis during an erection. The reason behind this is an enzyme called PDE5. PDE5 inhibitors that are taken orally will increase the blood flow to the penis and will also help maintain an erection over a longer period of time. 

When a penis is flaccid, the erectile tissue does not receive enough blood because the blood vessels are constricted. When aroused, the so-called cyclic guanosine monophosphate (cGMP) starts to flow. This leads to a relaxation of the vascular muscles and more blood will flow into the erectile tissue, resulting in an erection. PDE5 inhibitors have their effect by blocking the phosphodiesterase type 5 enzyme (PDE5). This enzyme reduces the cGMP and thus has an influence on the ability of the penis to be erect. The intake of PDE5 inhibitors increases the concentration of this messenger substance and the erection will last longer. However, PDE5 inhibitors only have an effect during sexual arousal. 

Medication based on PDE5 inhibitors has generally proven to be very reliable. The most-known brands are Viagra and Cialis as well as a range of other drugs available on prescription. Apart from PDE5 inhibitors, there are further alternatives available on the market which can also help treat erectile dysfunction. However, these are not offered by DoktorABC.

PDE5 inhibitor medication

PDE5 inhibitors are a group of drugs that are used for the treatment of erectile dysfunctions. In the UK, two different PDE5 inhibitors are approved for use.1

Erectile dysfunction (ED) occurs when an insufficient amount of blood flows into the penis. Most commonly this is due to an enzyme called phosphodiesterase type 5 (PDE5). In most cases, your general practitioner will recommend for you to take PDE5 inhibitors. These block the PDE5 enzyme your body produces, which causes the blood vessels to expand. The erectile tissue of the penis then fills up with blood more easily. The tablets are taken orally, and the effect lasts between four to 30 hours. Yet, the erection-enhancing effect only occurs if the man is sexually aroused. 

PDE5 inhibitors thus neither influence the libido of the treated person, nor can they cause an erection "automatically". 

Viagra is probably the best-known medication to treat erectile dysfunction. Its active substance, Sildenafil, has been prescribed for the treatment of permanent erectile dysfunctions since 1998. In 2003, an additional two substances entered the market: Tadalafil (Cialis) and Vardenafil (Levitra). The latest substance of this kind is Avanafil, also known under its marketing name Spedra. This substance was cleared for use in the UK in 2014.

The most known and used method of treatment for erectile dysfunction is oral tablets. For many, tablets are the preferred treatment because they are generally easier to take and effective (based on clinical trials). This only applies to prescription based PDE5 inhibitors. There are four Main PDE5 inhibitors used in erectile dysfunction medications.

Sildenafil and Tadalafil are the most-used substances for erectile dysfunction treatment. They are both categorised as PDE5 inhibitors and their erection enhancing effects both start soon after taking the pill. 

After the patent protection of the original Viagra medication by Pfizer expired in 2013, many cheaper generic medications have entered the market.

Could there be side effects?

Just like any other prescribed medication, PDE5 inhibitors also have potential side effects. 

Amongst the most common side effects of PDE5 inhibitors are headaches, skin redness in your face or on your upper body, or a stuffy nose. Your general practitioner will inform you about potential side effects.

When should you not take PDE5 inhibitors?

In some cases, PDE5 inhibitors are not recommended. Please always check with your general practitioner before you start any medication. 

If you are suffering from severe heart, circulatory or liver problems, or have had a stroke within the last six months, you should not take any PDE5 inhibitors. The same applies to certain eye conditions. 

Furthermore, there are a variety of medications that emit nitrogen monoxide. Medics call these NO donors. When PDE5 inhibitors and nitrogen monoxide are combined, the blood pressure in your body can fall dramatically. Worst case, this can be life-threatening. 

It is generally possible that potency enhancers and other medications influence each other's effectiveness.

Aside from the medicinal treatment of ED, some plants and natural substances can have positive effects on erectile function as well as the libido - your desire for sexual activity. Here are some well-known natural sexual enhancers. 

  • Gingko: The essence is won from the leaves of the Gingko tree. The tree initially came from China, but has now found a home around the world. The leaves contain flavonoid, terpenes, ketones and acids, which improve the arterial blood flow. 
  • Ginseng: One of the most popular plant-based substances, which has been used in China for thousands of years as an aphrodisiac.

Other methods of treatment

Injection therapy

With this method, medication is injected right into the penis. The penis stiffens up after about 20 minutes and remains erect for about 30 to 60 minutes. This method can only be prescribed by a urologist after a thorough diagnosis. It is not suited for patients that react negatively to PDE5 inhibitors or cannot take them. 

Penis pump

If you cannot take medications or you show allergic reactions to certain substances, there are more mechanical treatments of erectile dysfunction available. The most known method is the so-called penis pump. This is a vacuum pump that helps by pumping blood into the penis. 

Penis ring

The penis ring is applied similarly to the vacuum pump. This plastic ring is placed on the penis before sexual activity. An erection occurs due to increased blood flow into the erectile tissue of the penis. 

Surgical options 

If other conventional methods cannot be used or are not successful, there is the option of an implant into the erectile tissue. This method is reliable and only requires a single surgical procedure to enable an erection again.
