Previous generations of men with erectile dysfunction or impotence generally suffered in silence. The condition was seen as embarrassing and shameful, and there were only a few effective treatments available. A man’s self esteem could easily be crushed damaging intimate relationships, and destroy their quality of life. Fortunately, understanding of erectile dysfunction or impotence has been entirely transformed over the last few decades.

Viagra - The accidental erectile dysfunction treatment

In 1989 US pharmaceutical company Pfizer was working to develop a treatment for heart-related chest pains. During clinical trials they quickly discovered that the active ingredient Sildenafil had some interesting and unexpected side effects. The most striking of these was patients’ ability to achieve firm and long lasting erections. The new medication was given the brand name Viagra and marketed as erectile dysfunction tablets. By 2016, Viagra was being prescribed to almost 3 million men in the UK alone.

The success of Viagra, and now its generics, have revolutionised how we view erectile dysfunction in men and brought the issue out into the open. Men may still feel embarrassment, but are more willing to seek treatment and improve their lives.

The internet offers a wealth of information and resources, as well as the chance to order prescriptions for erectile dysfunction treatments online.

The erectile dysfunction treatment revolution

The commercial success of Viagra sparked a revolution in research and development that led to the marketing of several new brands based on different PDE-5 inhibitors. The various brands all work to overcome ED by increasing the flow of blood to the penis, but have a range of durations, onset times and side effects.

The Active Ingredient: PDE - 5 inhibitors

There are currently four main PDE - 5 inhibitors (Phosphodiesterase type 5 inhibitors) in use as the active ingredients of erectile dysfunction medication:

  • Sildenafil
  • Tadalafil
  • Vardenafil
  • Avanafil

How do PDE - 5 inhibitors work?

In order to achieve an erection, the volume of the blood in the penis has to increase by up to 40%. When a man becomes sexually aroused, the extra blood flows into the penis through its blood vessels. Once there, the blood needs to remain in place until you ejaculate. As soon as the blood flows back out the erection will be lost.

If a man’s body is functioning correctly, it releases cGMP when sexually aroused. This enzyme helps dilate blood vessels and relax the muscles in the penis allowing the corpora cavernosa (sponge like tissues of the penis) to fill with blood. It also prevents blood from flowing back out of the penis.

The phosphodiesterase-5 enzyme (PDE-5) works to break down the cGMP when it is time for the erection to subside. If this enzyme is over-produced or released too early, either preventing an erection or keeping one for long enough to have penetrative sex. PDE-5 inhibitors restrict the action of the PDE-5 enzyme and helps men enjoy a normal sex life.

What is the difference between generic and branded tablets?

Branded erectile dysfunction medications are simply the first medicine to have used a particular PDE-5 inhibitor as an active ingredient. The parent company or manufacturer invested in research and development and testing and succeeded to obtain licensing and approval for the new medication.

Branded erectile dysfunction medications available at Doktor ABC include:

  • Viagra
  • Cialis
  • Cialis Daily
  • Levitra
  • Spedra

Each branded medication uses a specific PDE -5 inhibitor as an active ingredient.

Branded Medication Active Ingredient
Viagra Sildenafil
Viagra Connect Sildenafil
Cialis Tadalafil
Cialis Daily Tadalafil
Levitra Vardenafil
Spedra Avanafil

Pharmaceutical companies usually file for a patent on their new medication but they do not retain exclusive rights indefinitely. After 15 years, when the patent runs out, other companies are allowed to manufacture medications that use the same active ingredients as the branded medications.

The new generic medications are basically a copy of the branded medication, but they use the name of the active ingredient as a brand name.

e.g. you could buy the branded medication Viagra (manufactured by Pfizer) or a generic version marketed under the name Sildenafil by rival companies, such as Teva and many others.

  • Generic medications sold in the UK are subject to exactly the same licensing and regulation as branded medications.
  • Generic medications may be cheaper because the manufacturer did not have to invest in the R&D process, testing or initial marketing.

Can I get ED medication on the NHS?

A few erectile dysfunction treatments are currently available on the NHS, but their availability is strictly limited to certain categories of patients, and the rules governing their prescription may be subject to change. Branded ED medications are only available in a limited number of circumstances.

Are ED pills available over the counter?

Original erectile dysfunction tablets can only be obtained with a prescription. It is extremely dangerous to buy ED pills over the counter for several reasons.

First off, it is illegal and you do not know what you are getting. Counterfeit medicines may contain unknown, untested and potentially dangerous ingredients.

Secondly, Erectile Dysfunction medications may conflict with other medications you are taking, or your health issues may make it inadvisable to take certain medicines. A physician needs to look at your medical history and current prescriptions and determine what medicine, if any, is right for you.

It is important to consult with a licensed physician or alternatively, an online GP at DoktorABC who can answer all the questions honestly and prescribe a medication which is safe for you to use.

How do I choose the right erectile dysfunction treatments?

There are several proven erectile dysfunction treatments on the market. Most men have heard of Viagra, but may be less familiar with other medications that use PDE-5 inhibitors. It’s important to choose the right medication. Our GP will evaluate your medical questionnaire carefully to make sure that you receive a safe and effective treatment that meets your individual health requirements. It may help if you also have some insight into the diagnostic process.

Questions to ask yourself before requesting a specific erectile dysfunction treatment:

  • How often do I intend to have sex?
  • How long do I want each tablet to last for?
  • How many ED tablets do I need to order?
  • Do I prefer a branded erectile dysfunction medication or a generic medication?
  • Do I have underlying medical conditions that may affect my suitability?
  • Are there indicators that I may suffer from side effects with a specific treatment?

If you are unable to answer the last two questions, our online questionnaire will help you focus on the information needed by the GP to evaluate your suitability. Our online GP will consider your preferences carefully when he or she evaluates your medical questionnaire and checks whether there are any health issues that may affect your treatment.

What to consider when choosing a prescription service

  • Do I need my prescription urgently?
  • Will I need repeat prescriptions over a long period?
  • Is it convenient for me to see a GP and go to the chemists?
  • Am I putting off seeking treatment through shyness or embarrassment?
  • How important is discretion and privacy to me?

If you answered yes to any of the above questions, Doktor ABC can offer a fast, discreet and convenient prescription renewal service. All you have to do is complete a short online medical questionnaire in complete confidence. This will be immediately reviewed by one of our online GPs. Once your prescription is approved, your order will be sent for discreet next day delivery by UPS courier. Checkout prices include your prescription, medication, and delivery.
